The construction certificate inspectors actually read.
Annexure 3 is the form named in Construction Regulations 2014 regulation 7(8). The employer declares the job and the exposures. Our occupational health practitioner completes the medical and issues the certificate of fitness.

Annexure 3 done wrong
A form that does not match the worker's real risks gets rejected at audit. Our practitioners test against the job and exposures your employer section declares.
Certificate missing at the gate
No valid Annexure 3, no site access. We schedule medicals around your start dates so the file is complete before the first shift.
Paper that does not survive the audit
The law keeps medical records for years. Your Care Net certificates sit in My Clinic Online as well as the site file, ready for the inspector and for access requests.
Book Annexure 3 medicals
Download the form, complete the employer section, and send the job list. We do the medical.
Construction Regulations 2014, form by form
Annexure 3 is not a Care Net invention. It is the form the regulation names. The rest of the Construction Regulations tell you what that form must support on site.
The certificate itself
A contractor must ensure that every employee has a valid medical certificate of fitness, specific to the construction work to be performed, issued by an occupational health practitioner in the form of Annexure 3.
General workers, teams at height and operators each need the form completed for their category, not a photocopy of someone else's.
Read regulation 7 in the gazetteThe risk assessment leads
Before work starts, a competent person must do a risk assessment of the construction work. The risks it identifies are the same risks your Annexure 3 forms must declare.
If the assessment says noise, chemicals or height and the form is blank, the inspector has a finding.
Read regulation 9 in the gazetteFall-risk names on the form
Your fall protection plan must include the process for evaluating medical fitness and the records of that process. Those records are the heights Annexure 3 certificates.
Use the working at heights medical for anyone the plan places in a fall-risk position.
Read regulation 10 in the gazetteThe health and safety file
Regulation 7 requires the contractor to keep a health and safety file on site. Annexure 3 certificates, fall protection fitness records and operator certificates live in it.
When a Department of Employment and Labour inspector arrives, that file is the first thing they open.
The Annexure 3, explained simply
Before the medical, the employer completes an Annexure 3 form. It lists the worker's job, the risks and exposures on your site, and the protective equipment the job needs. Our practitioner then tests against that form and issues the medical certificate of fitness, including fit with restrictions or fit with conditions where that applies.
Not sure how to complete it, or which category a worker falls into? Ask us. That is what we do every day.
