Pre-employment medicals

A certificate of fitness before the first shift.

A pre-employment medical confirms that a new starter is fit for the actual job and exposures, not a generic checklist. Care Net tests against the Occupational Health and Safety Act 85 of 1993 and the risks you declare.

Why this medical comes first

Fit the person to the job, not the other way round

Stopped at the gate

A missing pre-employment certificate keeps a new starter off site and your programme behind. We schedule around your start dates.

Wrong tests for the role

A generic medical that ignores noise, chemicals or height is not a defence. We test against the job description and the exposures you declare.

No baseline to compare later

Periodic and exit medicals only work if you recorded the starting point. The pre-employment file is that baseline, stored digitally for the legal retention period.

The law in plain language

What the OHS Act asks before day one

Section 8 of the Occupational Health and Safety Act 85 of 1993 places a duty on every employer to provide a working environment that is safe and without risk to health. A pre-employment medical is how you show that duty was considered before the person started.

OHS Act 85 of 1993, section 8

The general duty

You must identify hazards, assess the risk and take steps that are reasonably practicable. For a new starter, that includes confirming medical fitness for the actual work.

On your site

Send the job description and the risk assessment with the booking. The practitioner tests that person against that job, not a blank form.

Construction Regulations 2014, Annexure 3

Construction and related work

If the work is construction work, regulation 7(8) requires a medical certificate of fitness issued by an occupational health practitioner in the form of Annexure 3, specific to the construction work to be performed.

On your site

Complete the employer section of the Annexure 3 before the medical. We complete the medical section and issue the certificate.

Hazardous Chemical Agents Regulations 2021

Chemical exposure from day one

Where the role will involve hazardous chemical agents (the current HCS framework), medical surveillance starts with a baseline before exposure begins, not after the first spill or shift in the store.

On your site

Tell us the agents on the safety data sheets. The panel is matched to those agents and confirmed by the practitioner.

Noise Exposure Regulations 2024

Noise-exposed starters

Where the job sits in a noise zone, a baseline audiogram belongs in the pre-employment file. The Noise Exposure Regulations 2024 are the current noise framework. Hearing screening at Care Net follows SANS 10083.

On your site

A later periodic test is only useful if you have this first reading to compare. Do not skip the baseline to save a day.

How a Care Net pre-employment medical runs

You send the job titles, start dates and the exposures. We confirm the test set. The worker attends a fixed clinic, or a mobile clinic comes to your site. The record lands in My Clinic Online so the certificate verifies online.

If the role is at height, use the working at heights medical. If you already have a workforce on surveillance, start them on the medical surveillance programme after this baseline.

Quick answers

Frequently asked questions

Is a pre-employment medical required by law in South Africa?
The Occupational Health and Safety Act requires you to provide a working environment that is safe and without risk to health. Where the job carries identified exposure or a fitness requirement, a pre-employment medical is the baseline. Construction work also needs a medical certificate of fitness in the form of Annexure 3 under the Construction Regulations 2014.
What does a Care Net pre-employment medical include?
A baseline questionnaire, the physical checks the job needs, and tests matched to the declared exposures. Hearing follows SANS 10083. Spirometry follows the South African Thoracic Society guideline. Vision, blood pressure and other checks are added when the role requires them. An occupational health nurse practitioner reviews every medical.
When should the medical happen?
Before the worker starts the job, not after the first shift. For construction, working at heights and operator roles, the certificate must be valid before the worker is placed. Book the clinic or a mobile unit around your start date.
How long is the certificate valid?
Usually one year, but the practitioner can set a shorter period based on the worker's health and the risks of the job. The expiry date is printed on the certificate and can be verified on My Clinic Online.
Can one worker walk in, or do you only take teams?
One person is enough. Send a single contractor to Midrand, Elandsfontein, Brackenfell or Gqeberha, or book a mobile clinic once you have a team of ten or more on one site.
